Case Management Services
Case Management Services - implement individually centered, family, and community focused services whereby consumers are assisted in identifying, securing, and sustaining resources necessary to live in the community. Our model is trend setting in Iowa and prides itself on integrating case management services with all other services provided and is funded through a sub-contract with the Polk County Board of Directors and is accredited by the State of Iowa. Link Associates provides the comprehensive advocacy and support necessary for consumers eligible for Title XIX, as well as for the consumers we serve who are not eligible.
Each caseworker has a caseload small enough to ensure personalized services for the consumers and their family. In addition to the provision of case management, this department also serves as the community resource and referral contact. Link has three levels of case management support for consumers across Iowa.
• Case Management provides coordination of the Comprehensive Consumer Service Plan, crisis intervention, maintenance of consumer records, monitoring of on-going services, comprehensive advocacy and support for consumers eligible for Title XIX.
• Service Coordination includes assessment of service needs, including assurance that a diagnosis and evaluation is obtained for each consumer; development, with an interdisciplinary team, of an appropriate individual program plan for each consumer; assisting the consumer in obtaining needed services; monitoring the provision of services and the consumer’s progress; and, advocating on behalf of the consumer.
• Program Management assists consumers receiving day and/or residential services from Link Associates, but have an external Case Manager in the development of the Comprehensive Consumer Service Plan, crisis intervention, maintenance of consumer records, correspondence and all other required consumer data and advocates for consumers.
Any individual with developmental disability, chronic mental illness, or who is intellectually disabled and who is not in a Medicaid -funded institution is eligible. Link Associates works primarily with people who have an intellectual or developmental disability and have significant functional limitations. The focus of our case managers and service coordinators is to educate, advocate for, and help you to obtain appropriate services. They will also coordinate and monitor the continued appropriateness of those services. The state and county of legal settlement share equally the non- federal share of the cost for an adult’s covered service.
